its a random glitch that happens at any time.. hes an orc and a tauren mixed... check out his face. I notice it a lot with undead

Here we have Grimlos undergoing this very same phenomena. Strange how it seems to be plagueing the shaman class in specific. Notice the hole in his face underneath his lips, the pale colour of skin, the female looking facial features, and the bolts in his face. Totally undead.
